How do I keep the cotton from shifting in my futon mattress from shifting making it hammock like when slept on?

It makes a deep body impression from the cotton shifting. I have put several quilts folded up under the mattress pad to lessened the deep hole. I also noticed the mattress pokes through and puffs in between the wooden rungs on the bottom. Has nothing to do with the weight of the person, does this for everyone
